We have updated our Consumer Food Returns Form to try and streamline the process for our retailers, vendors & team members.

Please use the below form for each individual consumer food return. You can submit the form as many times as needed and it will email a copy of the form for your records.

All Provide, Blue Ridge Beef & Primal must be returned directly through the vendor.

Instead of mailing in UPCs - we are asking that you take a picture of the UPC and have the Consumer sign and date the UPC and submit a picture for vendor records.
